

Heat escaping from the living space into the attic. That heat warms the roof deck, snow on the upper roof melts, the meltwater runs down to the eave — which is unheated and below freezing — and refreezes there. Repeat that cycle for a few days and you have a ridge of ice at the roof edge with water pooling behind it.
Notice what is not in that description: gutters. The gutter is where you see the ice dam. It is not where the problem starts.
Because the correlation is real even though the causation is not. A guard adds a horizontal surface right at the eave, exactly where refreezing happens. On a home that already dams, that surface gives the ice something extra to form on and around, and the result looks worse than the same winter would have looked on an open gutter.
Tellingly, this drawback shows up even in favourable coverage: "a tendency to create ice dams during the winter months" appears as a listed negative inside otherwise-positive reviews of major micro-mesh systems.
Most guard content will not tell you there is such a case. There is.

If you get ice dams, the money goes further in the attic than at the eave:
Do those and the ice dam stops forming. Then a gutter guard is doing the job it is actually good at — keeping leaves out.
If a dam does form over a guard, clearing it may require removing the guard. Micro-mesh systems fastened into the fascia with hidden fasteners are not designed for seasonal removal, and a roofer charging to pull and re-set a section is a cost that never appears in the sales presentation.
Gutter guards do not cause ice dams; attic heat loss does. But if you already get ice dams, a guard will not fix them and can make the ice harder to clear. Spend the money on air sealing and insulation first, then buy the guard for the job it is genuinely good at - keeping leaves out.
